We also collect and store the information you provide on any of our online forms for any of our services or through our App to schedule an appointment or consultation. We store all data for no more than three (3) years.

Anyone under the age of 18 is expressly prohibited from submitting or posting any information on this Website. We do not knowingly collect personal information of anyone under the age of 18 and do not intend that any materials contained on this Website are directed to or targeted towards children under the age of 13.
